Celebratory and/or Memorial Donations

The Canadian Association for Parish Nursing Ministry is pleased to be able to offer a recognition plan for special times in a person’s life, i.e. Birthdays, Anniversaries, Baptisms, Confirmations, Weddings and of course Memorial recognition. Here is a chance to support the work of CAPNM and let the person you wish to honour or in the case of bereavement, the family of that person, know you are thinking of them.

As you are aware, CAPNM is a recognized charity and issues a charitable tax receipt for donations.

If you wish to make a donation in memory of someone, you are asked to e-mail Fran Hudson, RN, Parish Nurse, Coordinator of CAPNM at parishnursing@capnm.ca and indicate the following:

bullet1. The amount of the donation.
bullet2. The special event this is recognizing.
bullet3. Your name and mailing address.
bullet4. The name of the person being recognized.
bullet5. The name and mailing address of the person you wish notified that a donation has been made.
bullet6. Designate your donation to a specific use.
bullet7. Do you wish a Tax Receipt?

Once your information has been received, an appropriate letter will be sent immediately to the named recipient indicating that you have made a donation (no amount identified) in recognition of the event you have indicated.

Once your donation has been received by CAPNM, then you will receive a copy of the letter that was sent, and a charitable tax receipt, if so requested.

Thank you for your commitment to the work of CAPNM as it continues its work in supporting and establishing Parish Nurse ministries across Canada that emphasize the wholeness of the body, mind and spirit. Rooted in a vision of Christ as Healer, this ministry grows out of the belief that all faith communities are a place of health and healing and have a role in promoting wholeness through the integration of faith and health.
